Our Community Resource Navigator plays a vital role in supporting individuals and families by identifying emerging needs and connecting to local resources.  Additional services include:

• Refer to local and regional providers, and facilitate connections wherever necessary;

• Provide advocacy and/or support for self-advocacy;

• Assist with applications and assorted paperwork;

• Identify gaps in service and advocate for change;

• Follow-up on services provided to monitor progress and provide additional support when required.
